General Health Office Information:

Physical Exams & Immunization RecordsRequired upon entry or transfer into the school system and prior to the 4th & 7th grades.

Health Screenings- 

  • Vision screenings in grades pre-k through 5 and in 7th grade. 

  • Hearing screenings in grades K through 3 and in 7th grade. 

  • BMI (height & weight) screenings in grades 1, 4 and 7th. 

  • Postural screenings in grades 5 through 9. 

  • If your child does not meet the standards on these screenings, the nurse will send a letter home to inform you. You will be requested to have a follow up evaluation done by your child’s physician and provide the health department with documentation of the follow up.  

Medication Policy-

  • If your child requires medication (prescription or over-the-counter) during school hours the school nurse must receive a medication order form their provider and have a signed parent authorization form

  • If your child requires an inhaler for school the nurse should receive an asthma action plan from their physician. 

  • If your child requires an epipen for school the nurse should receive an allergy action plan from their physician. 

  • Medications including inhalers & epipens must be brought in by a responsible adult, they cannot be sent in with students. 

  • All medications must be in a labeled container. 

  • An over-the-counter parent consent form will be sent out annually for our standing order medications such as tylenol, motrin, benadryl, etc. 

Student Illnesses- 

  • Students will be sent home for fever of 100 or greater, vomiting, diarrhea, continued malaise after rest and quiet, discovery of head lice, suspected strep or any other medical or emergency conditions unable to be resolved within the framework of allowable treatments.

  • Head lice: Please contact the school nurse before sending your child to school.

  • Chicken pox: Child must be excluded for 5 days after the last eruption is crusted over or if a note is received from the child’s doctor.

  • Strep throat: Child may return after 24 hours of starting antibiotic. 

  • Conjunctivitis (pink eye): Child may return with a doctor’s permission 24 hours after treatment has begun. 

  • Influenza: Child may return once symptoms have improved and they are fever free x 24 hours without the use of fever reducing medications.  

  • COVID-19: Child is excluded from school x 5 days since symptom onset or positive test and must wear a mask 5 days upon return.
